Background: Home exercise program (HEP) is a basic part of rehabilitation for stroke survivors after stroke incidence. Studies have investigated the rate of adherence to HEPs among the stroke population to be generally dissatisfying. Previous studies have been carried out to determine the association between adherence to HEP and functional outcome among various patients population. However, report is lacking on the association between adherence to HEP and functional outcome among stroke survivors. Aim: The aim of this study was to determine the relationship between adherence to HEP and functional outcome among stroke survivors. Methods: A cross-sectional design was used to recruit 78 consenting stroke survivors from two major hospitals in Benin City. A 16-item Exercise Adherence Questionnaire was used to obtain information on socio-demographic data, adherence level, and reasons for adherence or nonadherence. Functional Independence Measure (Motor domain) was used to obtain information on level of independence. Data were analyzed via descriptive and inferential statistics. Level of significance was set at p<0.05. Results: Most participants, 47 (60.3%) were non-adherent to HEPs, while 31 (39.7%) were adherent. Adherence to HEP had significant association with gender (χ 2=5.233, p=0.022), and educational level (χ 2=7.870, p=0.020). There were significant positive relationships (p<0.05) between HEP adherence and adjusting exercise to suit self, exercise is fun, family and social support, and exercise is beneficial. Significant negative relationships (p<0.05) were observed between adherence to HEP and other commitments, lack of time, lack of exercise understanding, tiredness, and pain. No significant association was found between adherence to HEP and functional outcome. Conclusion: Adherence to HEPs was poor in this study. Female and less educated stroke survivors showed a high likelihood of non-adherence. Numbers of factors were revealed to have significant negative influence on adherence to HEP. Several factors were linked to facilitate adherence to HEP. Adherence to HEP was not found to yield good functional outcome among stroke survivors in this study.
